Polder Health Care is committed to encouraging social inclusion for all residents and actively encourages use of community resources and activities wherever possible. This maximises social and community interaction and meaningful activity, identified as positively beneficial for a range of mental health presentations (Department of Health, 2011)

Activities, outings and one two one interaction are available if requested and identified in the individuals care plan
Physical well-being

Regular meetings are held with residents to discuss issues relating to their personal care and they will require to be registered with a General Practitioner (GP) of their choice

Medical attendance by local general practitioners can be arranged or service users may retain their own doctor should they be from the local area

Service users are encouraged to visit a chiropodist, dentist and optician periodically. Alternatively, we can arrange for home visits where appropriate.
